出院计划模式在高龄永久性膀胱造瘘病人中的应用及效果评价
Application and effect evaluation on discharge planning model in elderly patients with permanent cystostomy
摘要
Abstract
Objective:To discuss the application effect of discharge planning model in elderly patients with per-manent cystostomy.Methods:By convenience sampling,a total of 89 patients were enrolled and randomly divided into intervention group and control group.The patients in control group received routine nursing and phone call following up;on the basis of routine nursing,the patients in observation group received continuity of care based on discharge planning model;patients in both of in groups received intervention for 6 months.Results:The gen-eral self care ability of patients in observation group was higher than that in control group(P<0.05).The in-cidence of complications and unplanned hospital admission rate in intervention group were lower than those in control group(P<0.01).Conclusions:Applied the discharge planning model for the elderly patients with perma-nent cystostomy improved the home care nursing ability and quality of life of patients and their families,and re-duce the incidence of complications and unplanned hospital admission rate.关键词
