Abstract
With development of economy,science and technology and development of culture and education,the focus of health management was shifting from hospital to family and community,from disease treatment to dis-ease prevention.Research on continuity of care based on WeChat platform was being paid attention to,this paper reviewed research progress on continuity of care based on WeChat platform in China,and summarized applica-tion effect and existing problems.关键词
