Abstract
Objective To quantitatively evaluate the effectiveness of unicompartmental knee arthroplasty (UKA) in the treatment of knee osteoarthritis(KOA) by using the meta-analysis method. Methods The related randomized controlled trials (RCTs) on UKA in treating KOA were retrieved from the CNKI,WanFang database,VIP database,PubMed,Embase,Cochrane Library,Web of Science by computer. The retrieval time limit was from their establishment to June 2016. Two valuators independently screened the literatures according to the formulated inclusion and exclusion criteria , extracted the data and evaluated the quality of included literatures . The Revman 5 . 3 software was used to conduct the meta-analysis on RCTs conforming to the standards. Results There were 13 included RCTs involving 811 patients (831 knees). The meta analysis showed that the operative time ,intraoperative bleeding amount and hospitalization time had statistical difference between the UKA group and the total knee arthropalstyotal (TKA) group [mean deviation (MD)=-3.00,95%CI-13.59~-2.57,P<0.01;stan dardized mean difference (SMD)=-9.98,95%CI-13.12~-6.83,P<0.01;MD=-2.01,95%CI-2.43~-1.59,P<0.01]. The needing time of knee flexion to 90 ° had statistical difference between the two groups (MD=-3.42,95%CI-3.77~-3.08,P<0.01). The excellent rate of pain relief had no statistical difference between the two groups (OR=2.02,95%CI-0.75~5.46,P>0.05),while the HSS score at postoperative 1 year and knee joint flexion at postoperative 1 year had statistical difference between the two groups (SMD=0.33,95%CI 0.15~0.52,P<0.01;MD=-10.65,95%CI 7.48~13.83,P<0.01). Conclusion According to the results of this meta analysis,in the early and middle stage treatment,UKA exhibits relatively satisfactory effect than TKA. But this conclusion is limited by the number and quality of included RCTs,which still needs large sample high quality RCTs to further verify.关键词
