腹腔镜下与开放式一期Fowler-Stephens手术治疗小儿腹腔型隐睾临床疗效比较
Comparison of Clinical Effect of Laparoscopic and Open Single-stage Fowler-Stephens Surgery ;for Pediatric Abdominal Cryptorchidism

Abstract
Objective:To compare the differences of curative effect between laparoscopic and open surgery single-stage Fowler-Stephens surgery for pediatric abdominal cryptorchidism.Method:100 pediatric abdominal cryptorchidism patients who were proposed for single-stage Fowler-Stephens surgery in our hospital from June 2014 to October 2016 were selected as research objects,according to the random number table method,they were randomly divided into the laparoscopic group and the open group,50 cases in each group.The laparoscopic group was taken laparoscopic single-stage Fowler-Stephens surgery,and the open group was taken open single-stage Fowler-Stephens surgery.The operation time,blood loss,postoperative hospital stay and hospitalization expenses were recorded and compared with between the two groups.The expression levels of high sensitive C reactive protein(hs-CRP),interleukin 6(IL-6)and interleukin 17(IL-17)of two groups of children were measured and compared with at postoperative 3 d and 7 d.The two group children’s parents’ rate of treatment satisfaction was given a questionnaire survey and results were compared with.After from 1 to 24 months,two group patients were diagnosed by abdominal ultrasound,and the incidence rates of postoperative complications were recorded.Result:The children of the laparoscopic group’s operative time,bleeding volume and duration of hospitalization were significantly lower than the children’s in the open group(P<0.05),two groups of children’s hospitalization expenses had no significant difference(P>0.05).The expression levels of hs-CRP and IL-6 of postoperative 7 d in the two groups after operation were significantly lower than those of postoperative 3 d (P<0.05),the expression levels of IL-6,IL-17 and hs-CRP of postoperative 3 d and 7 d in the laparoscopic group were significantly lower than those of the open group(P<0.05).The rate of postoperative complications of the laparoscopic group was significantly lower than that of the open group(P<0.05),the surgery success rate of the laparoscopic group was significantly higher than that of the open group(P<0.05).The parents’ satisfaction rate of the laparoscopic group was significantly higher than that of the open group(P<0.05),the parents of the laparoscopic group were significantly lower than that of the open group(P<0.05).Conclusion:Before surgery,pediatric abdominal cryptorchidism patients should be strictly detected on the basis of adaptation and contraindication to decide whether they could be taken a single-stage Fowler-Stephens surgery,and laparoscopic single-stage Fowler Stephens is the preferred type of operation.关键词
