Abstract
Objective:To compare the target volume,normal tissue protection and number of hops of the 3 dimensional-conformal radiotherapy therapy(3D-CRT),static intensity modulated radiation therapy(s-IMRT), volumetric modulated arc therapy(VMAT).Method:24 cases of rectal cancer were selected and divided into 3D-CRT group,IMRT group,VMAT group,8 cases in each group.The target volume coverage indicators included average dose,conformity index(CI),and homogeneity index(HI)of planning tumor volume(PTV), normal organs at risk(OAR)included the bladder,small intestine,colon,and bilateral proximal femurs. Result:The average PTV dose of IMRT group and VMAT group were higher than that of 3D-CRT group,and the target area conformal and uniformity were better than those in 3D-CRT group,the differences were statistically significant(P<0.05).The bladder,small intestine,colon Dmax,V40 of the IMRT group and VMAT group were significantly lower than those in 3D-CRT group,the differences were statistically significant(P<0.05).Compared with the 3D-CRT group,IMRT group and VMAT group were able to reduce the bilateral femoral head Dmax,V30,V40,the differences were statistically significant(P<0.05).Compared with the IMRT group,VMAT group can further reduce the small intestine of Dmax and bilateral femoral head Dmax,V30,V40(P<0.05).The number of machines of VMAT group and IMRT group were higher than that of 3D-CRT group,and the IMRT group was higher than the VMAT group,the differences were statistically significant(P<0.05).Conclusion:VMAT can improve target dose coverage,better protection of normal tissues in the small intestine and bilateral femoral head protection,VMAT is better than IMRT and VMAT,the treatment time is shorter,is conducive to precise radiotherapy and the protection of the linear accelerator,the future VMAT will be widely used in locally advanced rectal cancer after radiotherapy.关键词
