Abstract
Objective To observe curative effect of hypertonic sodium chloride hydroxyethyl starch 40 injection(HSH40)in the treatment of traumatic brain injury(TBI)combined with hemorrhagic shock and its brain protective mechanism.Methods The selected 45 TBI patients with hemorrhagic shock were randomly divided into three groups,1 5 cases in each group.Group A re-ceived Lactated Ringer’s solution(LRS)500 mL at the drip speed of 20 mL/min,group B received mannitol 250 mL at the drip speed of 1 5 mL/min and group C received HSH40 250 mL at the drip speed of 1 5 mL/min.All three groups were given first aid treatment.Thirty minutes after treatment,we observed and recorded the changes of mean arterial pressure(MAP),heart rate(HR) and Glasgow coma scale(GCS)scores.At the same time,we used the ELISA method to detect the expressions of serum S-100B, Nrf2 and HO-1.Results Before liquid resuscitation,MAP,HR and GCS showed no significant differences in three groups(P >0.05),but thirty minutes after treatment the former two indicators were improved in group A(P <0.05)while the GCS showed no obvious changes(P > 0.05 ).The group B presented better improvement after treatment in GCS scores (P < 0.05 ),but in neither MAP nor HR(P >0.05).In addition,MAP,HR and GCS scores in group C were significantly better improved after treat-ment(P <0.05).After treatment,the expressions of serum S-100B,Nrf2 and HO-1 showed no significant changes in group A and group B(P > 0.05 ),but disclosed significant changes in group C (P < 0.05 ).Conclusion During the period of emergency treatment for TBI combined with hemorrhagic shock,HSH40 has a rapid anti-shock effect.Meanwhile,HSH40 has a cerebral pro-tective effect,which may be related to activation of Nrf2 and up-regulation of the expression of HO-1.关键词
