单侧椎板切除双侧减压与传统PLIF减压手术治疗腰椎管狭窄症的临床疗效对比
Comparison of clinical efficacy of unilateral laminectomy for bilateral decompression operation versus traditional posterior lumbar interbody fusion operation for lumbar spinal stenosis

Abstract
Objective To compare the clinical efficacy of unilateral laminectomy for bilateral decompression( ULBD) operation versus tra-ditional posterior lumbar interbody fusion( PLIF) operation in the treatment of lumbar spinal stenosis( LSS) . Methods A retrospective analysis was performed in 33 cases of LSS treated with ULBD operation( ULBD group) and 41 cases treated with traditional PLIF operation (PLIF group) from March 2014 to March 2015. The clinical indicators were compared between two groups, including actual operation time,intraoperative blood loss, volume of postoperative negative pressure drainage,postoperative bed time,visual analogue scale(VAS), JOA score of the lumbar vertebra,and postoperative X-ray or CT scan of lumbar vertebra. Results All patients were followed up for 12-24 months. There was no significant difference in actual operation time between two groups(P>0. 05). The intraoperative blood loss, volume of postoperative negative pressure drainage,postoperative bed time in ULBD group were all significantly lower than those in PLIF group(P<0. 05). The VAS and JOA score of lumbar vertebra showed no significant difference at 5 d after the operation between the two groups(P>0. 05). VAS score in ULBD group was lower than those in PLIF group at 12 month after the operation(P<0. 05), while JOA score was significantly higher. Both the X-ray and CT scan of the lumbar vertebra showed that the fusion rate in the intervertebral space had no significant difference between the two groups(P>0. 05). Conclusion Unilateral laminectomy for bilateral decompression operation is safe and effective in the treatment of LSS. It has lower intraoperative blood loss, postoperative suction drainage,postoperative bed time than the traditional PLIF decompression surgery. Moreover, it also has the advantages of minimal injuries, slowing down the de-generation rate in vertebral bodies between adjacent spinal segments and decreasing the possibility of secondary instability of the spine and low back pain after operation.关键词
