老年股骨粗隆间骨折术后早期应用唑来膦酸治疗骨质疏松的临床疗效

摘要
Abstract
Objective To observe the clinical efficacy of early application of zoledronic acid after operation of femoral intertrochanteric fracture in osteoporotic patients .Methods Seventy-nine old patients with osteoporotic femoral intertrochanteric fracture , who were treated in our hospital from May 2013 to October 2014 , were selected .All patients were randomly divided into control group and treatment group .The treatment group had 38 patients , and the control group had 41 patients .Patients in the treatment group received an intravenous zoledronic acid 5 mg in 3-7 days after operation and once a year since then .The non-steroidal anti-inflammatory drugs were also given simultaneously .Patients in the control group received 100 mL of intravenous normal saline instead .All the patients took 600 mg Caltrate D twice daily and 0.5 μg alfacalcidol once a day .Visual analogue scale ( VAS) and lumbar bone mineral density was measured , and recurrent osteoporotic fracture incidence were recorded .Results Except for two patients in the control group died of other diseases during the follow-up, the remaining 77 cases completed a 12-18 months clinical follow-up, with an average of 13.45 months.Compared to the control group, one year after treatment VAS 0.05-3.5, hip pain and VAS were signifi-cantly improved in zoledrnic acid group ( P<0.05 ) , and BMD in treatment group after 1 year increase 0.06 -0.15 g/cm2 , the average increase of 0.11 cg/cm2 , BMD was significantly increased ( P<0.05 ) and refracture incidence was decreased .Healing time of fracture had no difference between two groups .Conclusion Zoledronic acid can relieve hip pain, increase BMD and reduce recurrence of osteoporotic fracture , therefore, which is worth being applied after op-eration of femoral intertrochanteric fracture in aged osteoporotic patients .关键词
