Abstract
Objective To investigate the clinical value of gluteus maximus end tendon extended reconstruction in treating severe gluteal muscle contracture. Methods Forty-six patients with gluteal muscle contracture were divided into two groups. In group A, 26 patients (46 hips) underwent gluteal muscle contracture, then the gluteus maximus was extensively reconstructed by Z-shaped of the gluteus maximus end tendon, and 20 cases (33 hips) in group B were treated with gluteal muscle contracture. The postoperative flexion and extension of the hip, hip adduction and in-ternal rotation, extensor strength and exercise capacity of the two groups were compared. Results All 46 patients were followed up for 1~5 years. There was significant difference between the two groups (P<0. 05). There was no significant difference between the two groups in terms of knee flexion and hip flexion, hip adduction and internal rota-tion activity, extensor strength and exercise capacity. In group B, there were 8 cases of gluteus muscle strength gradeⅢ, of which 3 cases (5 hips) complained that the gluteus muscle weakness and the posterior tread were weak after sitting, muscle strength were in gradeⅣor more, the effect was better than group B(P<0. 05). Conclusions For the patients with severe gluteal muscle contracture, the traditional gluteal muscle contracture with loosening of the glu-teus maximus while the end point tendon for Z-shaped extension reconstruction, not only can make the gluteus con-tracture maximum release, muscle strength is well reconstructed and protected, and the curative effect is satisfactory.关键词
