地屈孕酮预处理及黄体支持对PCOS患者促排卵结局的影响
Effect of pretreatment and luteal phase support with oral dydrogesterone on outcome of induced ovulation in patients with PCOS

Abstract
Objective:To determine the effects of pretreatment and luteal phase support with oral dydrogesterone on the clinical outcome in patients with poly ovarian syndrome(PCOS).Methods:A total of 197 infertile women with PCOS were administrated with letrozole(LE) for inducing ovulation and given oral dydrogesterone for pretreatment and luteal support in the Women and Children Health Care Center in Dalian from January 2015 to June 2016.The patients were randomly divided to three groups:the patients were pretreated with dydrogesterone in luteal phase before induced ovulation cycle,and administrated with dydrogesterone for luteal support after ovulation in group A(n=56);the patients were administrated with dydrogesterone for luteal support after ovulation only in group B (n=71);the patients were administrated with progesterone capsule for luteal support after ovulation in group C(n=70).The general data,ovulation induction and pregnancy outcomes were compared among the three groups.Results:The incidence of luteinized unruptured follicle syndrome(LUFS) in group A(5.36％) was significantly lower than that in group B(11.27％) and group C(10.00％),but the endometrial thickness on HCG day in group A [(10.58±1.99)mm] was significantly thicker than that in group B [(8.75±1.14) mm] and group C [(8.12±1.15)mm](P＜0.05).Furthermore,the ectopic gestation rate in group A(0％) was significantly lower than that in group B(2.81％) and group C(2.85％).The vaginal bleeding rate and abortion rate in early pregnancy in group C(27.14 ％,11.43 ％)were significantly higher than those in group A(14.29％,5.36％)and group B(14.28％,5.63％) (all P＜0.05).The number of dominant follicles,ovulation rate,clinical pregnancy rate,and multiple pregnancy rate were not significantly different among the three groups(P＞0.05).Conclusions:Oral dydrogesterone pretreatment combined with luteal support can effectively improve endometrial thickness and reduce LUFS incidence and ectopic pregnancy rate in the cycles induced ovulation with letrozole in PCOS patients.It is still needed a larger samples of randomized controlled studies to confirm the results.关键词
