Abstract
Objective:To investigate the diagnostic value and clinical significance of various androgen indexes to polycystic ovary syndrome ( PCOS ) . Methods:The clinical mani-festations and biochemical androgen indexes in 294 PCOS patients and 116 female control sub-jects were analyzed,and the PCOS group was divided into four subgroups:high androgen,non-androgen,obesity and non-obesity,to detect the total testosterone(TT),sex hormone-binding globulin(SHBG),dehydroepiandrosterone sulfate(DHEA-S) and androstenedione(A4) in the patients,and calculate the free androgen indexes( FAI) to plot an ROC curve,and compute the area under curve ( AUC) ,and discuss the diagnostic value of various indexes to PCOS and its clinical manifestations. Results:BMI, mFG, acne, TT, DHEA-S, A4 and FAI scored higher in the PCOS group than the normal control group ( P<0 . 05 ) , while SHBG scored lower ( P<0 . 05 ) . According to the comparison of AUC between the PCOS group and control group:DHEA-S(0. 643)<SHBG(0. 746)<TT(0. 778)<A4(0. 849)<FAI(0. 915),there was a sig-nificant difference between TT and DHEA-S,A4 as well as FAI(P<0. 05),while there was no significant difference between TT and SHBG ( P>0 . 05 ) . According to the comparison of AUC between the high androgen subgroup and non-androgen subgroup:DHEA-S ( 0 . 638 ) <TT(0.685)<A4(0. 702)<SHBG(0. 847)<FAI(0. 938),there was a statistically significant difference between FAI and TT ( P<0 . 05 ) . According to the comparison of AUC between obesi-ty subgroup and non-obesity subgroup:DHEA-S ( 0 . 687 )<TT ( 0 . 711 )<A4 ( 0 . 739 )<SHBG (0. 768)<FAI(0. 860),there was a statistically significant difference between FAI and TT (P<0. 05). Conclusions:The diagnostic value of A4 and FAI to PCOS is higher than TT,and that FAI has higher sensitivity to the clinical manifestations of high androgen, as well as obesity, which makes it more suitable for clinical expansion and application.关键词
