Small-area estimation of health expenditure per capita in Shaanxi health service survey

Abstract
Objective To estimate the health expenditure per capita of the residents in specific areas of Shaanxi Province based on the national health service survey data. Methods Small-area Fay-Herriot model was built to estimate the annual health expenditure per capita in 32 sample counties / districts of Shaanxi province and three regions (Northern Shaanxi, Guanzhong, and Southern Shaanxi), as well as that of urban and rural residents and any possible errors, and analyze the relevant influencing factors. Results The estimated urban and rural health expenditure per capita in Shaanxi Province was CNY 625 and CNY 463; the health expenditure per capita in Northern Shaanxi, Guanzhong, and Southern Shaanxi was CNY 596, CNY 520 and CNY 473, respectively. The main influencing factor was the difference among the urban and rural areas and that among the different regions (mainly Northern Shaanxi, Guanzhong, and Southern Shaanxi regions). The absolute value of log likelihood function fit in Fay-Herriot model was 37.18, 66.19, and 10.88, respectively.Conclusion Small-area direct estimation and Fay-Herriot model fitting can be used to estimate the health expenditure per capita in a specific area. By use of the regional and urban-rural difference model, the health expenditure per capita of Shaanxi residents can be calculated accurately, which can provide reference for regional health management decision making and other similar studies.关键词
