长节段固定融合治疗退变性脊柱侧凸的中期疗效

Abstract
Objectives To evaluate the medium-term curative effect of surgical long segmental fixation and fusion in degenerative scoliosis. Methods From January 2007 to December 2012, 25 DS patients underwent long segmental fixation and fusion were colleted. Visual analogue scale (VAS) scores, Japanese Orthopaedic Association (JOA) score, Lumbar lordosis angles, Coronary Cobb angles and Postoperative complications were used to evaluate the clinical results. Results Dural tear occurred in 4 cases of all patients and was repaired during operation. All incisions primarily healed without infection. After implants removed, 3 cases had screw loosening or fracture. After symptomatic treatment, 1 patient had late healed, bone graft and fusion, two patients had adjacent segment disease 2 years after surgery, no adverse event happened after revision surgery. All patients were followed up 2-7 years (mean, 3.8 years). VAS and JOA scores were significantly improved at last follow-up compared to the preoperative values ( = 14.235 and -11.244, = 0.000). Coronary Cobb angles were significantly improved three months postoperatively ( = 12.329, = 0.000), but ≥ 1 year after surgery there was no further significant improvement compared to the preoperative values ( = 1.106, =0.280). The lumbar lordosis angle was not significantly changed postoperatively ( = -1.932, = 0.065). Conclusions Long segmental fixation and fusion in degenerative scoliosis could not only achieve full decompression, but also cover the entire curve to achieve the purpose of fixing and reconstruct spinal stability, which prevents the progression of degenerative scoliosis. Favorable medium-term curative effects can be achieved by long segmental fixation and fusion.关键词
