关节镜治疗症状性膝关节骨关节炎的疗效
Curative effect analysis of arthroscopic treatment for symptomatic knee osteoarthritis

Abstract
Objective To observe the clinical effects of arthroscopy in the treatment of knee osteoarthritis (OA) according to X-ray grading and cause of disease. Methods In this retrospective study, patients with a primary diagnosis of knee OA treated by arthroscopy between January 2015 and December 2015 were collected. Patients were ranked according to Kellgren Lawrence (KL) grading system. Different surgical procedures, such as synovitis cleaning, meniscectomy, removal of loose bodies and the micro fractures were recorded by reviewing operation video. Patients were followed up according to X-ray grading and cause of disease. The efficacy of arthroscopy for knee OA was evaluated by subjective satisfaction and Lysholm knee score. Results According to Kellgren Lawrence (KL) Grading System, postoperative satisfaction of patients with different grade was significantly different ( <0.05). The postoperative Lysholm score of patients with grade 0, grade Ⅰ and gradeⅡ was significantly different compared with the preoperative one ( <0.05). The postoperative score of patients with grade Ⅲ was not significantly different compared with the preoperative one ( >0.05). The postoperative score of patients with synovial hyperplasia, meniscus tear and joint loose bodies was significantly different compared with the preoperative ( <0.05). The postoperative score of patients with cartilage stripped was not significantly different compared with the preoperative ( >0.05). Conclusions Arthroscopy is an ideal treatment for knee OA with X-ray grading no more than grade Ⅱ and caused by synovial hyperplasia, meniscus tear and joint loose bodies. Arthroscopy for knee OA with X-ray grading more than grade Ⅱ and caused by cartilage stripped are unsatisfied.关键词
