Abstract

Objective:To research and evaluate the application of integrated head-body board in craniospinal irradiation (CSI).Methods: To retrospectively analyze the treatment results of craniospinal irradiation for 43 patients (21 cases were children patients, 0-14 years old; 22 cases were adult patients, >14 years old) used integrated head-body board. During therapeutic process, the registration of the scan image and digitally reconstructed radiograph were applied, which included three parts: head and neck segment, chest segment and lumbosacral segment. And the position errors of X-axe, Y-axe and Z-axe were adjusted by automatic registration. The data of positioning error during the treatment were analyzed by SPSS 22.0.Results: All of positioning errors in patient using head-body board were less than clinical setting error range(head-neck 0.3cm, body 0.5cm), X direction errors of three segments were (0.122±0.120), (0.074±0.211) and (0.083±0.096) cm, respectively; Y direction errors of three segments were (0.202±0.154), (0.236±0.175) and (0.283±0.187)cm, respectively; Z direction errors of three segments were (0.136±0.127), (0.249±0.472) and (0.272±0.211)cm, respectively; and all of the differences between every positioning error of every segment and corresponding clinical setting error had statistical significance: head-neck segment(t=-13.091,t=-5.598,t=-11.423;P<0.05); chest segment(t=-17.814,t=-13.325, t=-4.699;P<0.05); lumbosacral segment(t=-33.632, t=-8.974,t=-8.390;P<0.05). All of the differences of head-neck and spinal segment between children and adults didn't have statistical significance: head and neck segment(t=0.698,t=1.028, t=1.691;P>0.05), spinal segment(t=1.376,t=0.469,t=1.179;P>0.05); the comparison between any two means of X-axes for different segment between children and adults didn't have statistical significance (F=2.035,P>0.05), while Y-axes and Z-axes had statistical significance (F=3.777,P=0.024;F=3.895,P=0.022).Conclusion: In craniospinal irradiation, using the new measure to fix the supine position can achieve lower positioning error, convenient operation, and more compliance and comfort, at the same time, this measure is suitable for both of children and adult. Therefore, it is worthy in popularization and application for the clinical work.
