前颅底广泛性骨折的手术策略
Operation techniques for extensive anterior skull base fractures
摘要
Abstract
Objective To investigate the operation techniques for extensive anterior skull base fractures.Methods The 68 patients of extensive anterior skull base fractures were enrolled.The epidual approach on extensive anterior skull base fractures with 34 cases were performed.The subdual approach on extensive anterior skull base fractures with 34 cases were performed.We evaluated a serial of case data at two weeks,1 month and 3 months after the surgery.Results The patients'vision and olfactory function by the subdual approach were remarkably better(all P<0.05).The cure of cerebrospinal fluid leak and the clearance rate of frontal lobe contusion and hematoma by the subdual approach were remarkably increased(all P<0.05).The intracranial infection and pain in the forehead by the subdual approach were remarkably decreased (all P<0.05).There are no difference such as the frontal lobe atrophy,meningocele and mucous cyst between epidual and subdual approach (all P>0.05).Conclusions There are significant advantages to the subdual approach on extensive anterior skull base fractures,such as vision and olfactory sensation function recovery,the clearance rate of frontal lobe contusion and hematoma,the cure rate of cerebrospinal fluid leak and a low rate of intracranial infection.关键词
