P-选择素和D-二聚体预测联合血塞通对术后门静脉血栓的预防作用研究
Preventive Effect on Portal Vein Thrombosis after Splenectomy in Patients with Portal Hypertension by Selective Using Xuesaitong within Gauging P-selectin and D-dimer

Abstract
Objective To explore the preventive effect on portal vein thrombosis after splenectomy in pa-tients with portal hypertension by selective using Xuesaitong within gauging P-selectin and D-dimer. Methods Thirty-seven patients with portal hypertension receiving splenectomy (or united disconnection of portalazygous venous collaterals) were divided into two groups randomly (19 cases in observation group and 18 cases in control group1 ), and gauging P-selectin and D-dimer in the first day after operation were monitored. According to the standard (P-sel was higher than 12% in splenectomy, higher than 15% in splenectomy united disconnection of portalazygous venous collaterals, D-D was higher than 500 ng/mL), Xuesaitong was used for clinical intervention in 12 cases chosen from observation group and 18 cases in control group1. Other 48 cases with portal hyperten-sion without clinical intervention were set as control group 2. SAS software was used for statistic analysis. Results The morbidity rate of PVT was 21.1%, 22.2%, 54.2% in observation group, control group 1, and con-trol group 2, respectively. The morbidity rate of PVT was no significant difference between observation group and control group 1. And there was significant different of the morbidity rate of PVT between control group 2 and other groups. Also, the morbidity rate of PVT was significant different between group 2 and 7 cases in obser-vasion group without clinical intervention. Conclusion Xuesaitong can be used in preventing PVT effectively in patients with portal hypertension receiving splenectomy (or united disconnection of portalazygous venous collat-erals). Selective use of Xuesaitong inject can acquire the same effect in prevention of PVT according P-sel and D-dimer monitoring.关键词
