Abstract
Objective:To investigate the prognosis and related factors of postoperative radiotherapy in patients with early endometrial cancer (EC).Method:From Sep.2012 to Sep.2016,250 cases of patients with stage I EC in Nanyang Central Hospital affiliated Nanyang Hospital of Zhengzhou University underwent hysterectomy surgery,adjuvant radiation according to the pathological stage for postoperative treatment.Postoperative adjuvant radiotherapy:simple vaginal irradiation in 108 patients (43.2%),43 cases of pelvic irradiation (17.2%).Combined irradiation in 99 cases (39.6%) and the survival rate was calculated by Kaplan-Meier method and analysis of prognostic factors by the Cox model.Result:The average follow-up time was (52.58 ±12.35) months,5 year follow-up rate was 86.4% (216/250),the total follow-up rate was 83.2% (208/250).250 patients,10 cases of recurrence of EC due to deterioration of death,8 patients died due to primary or secondary cardiovascular disease and other high-risk disease.Five years overall survival rate and progression free survival rates were 92.6% and 89.5% respectively.Five years the total recurrence rate,distant metastasis rate,local recurrence rate were 8.4% (21/250),6.4%(16/250) and 4.8% (12/250) respectively.Single factor analysis and multivariate analysis showed that the main factors affecting the overall survival were the lower uterine were segment involvement (P=0.014) and anemia before radiotherapy (P=0.009).The adverse reactions in mild to moderate,151 cases (60.4%) suffered from grade 1,2 acute radioactive enteritis,9 cases (3.6%) suffered grade 3 or above adverse reactions,24 cases (9.6%) appeared in other late adverse reactions.Conclusion:The first stage EC patients received adjuvant radiation therapy after the general clinical treatment effect is good,adverse reactions are mild.The incidence of anemia before radiotherapy and lower uterine segment involvement are the main factors affecting the overall survivalrate.关键词
