Abstract
Objective To explore change of plasma Fbg and pulmonary function in chronic obstructive pulmonary disease (COPD), and analyze the correlation with clinical effect and pulmonary function. Methods Forty patients with ACOS (ACOS group) and 30 patients with COPD (COPD group) who received treatment in the department of respiration, and 30 healthy controls (control group) in the Sixth People's Hospital of Shanghai Jiao Tong University from January 2013 to June 2016 were selected as study subjects. All the patients were given long acting B agonist (LABA), inhaled corticosteroids (ICS) for 4 weeks. Plasma Fbg, pulmonary function were detected for ACOS, COPD before and after treatment and control group, and the correlation between level of Fbg and pulmonary function was analyzed. Results Compared with before treatment,indicators of pulmonary function in COPD and ACOS group after treatment were improved obviously (COPD group: t=2.234,t=5.911,t=6.176,P=0.034,P=0.000,P=0.000;ACOS group:t=3.222,t=5.134,t=4.392,P=0.010,P=0.000,P=0.004), and level of Fbg was decreased significantly (COPD group:t=5.183,P=0.000, ACOS group:t=4.262,P=0.005). All the indicators of pulmonary function in ACOS group were significantly lower than those in COPD group, and level of Fbg was significantly higher than that in COPD group(t=4.283,t=3.137,t=3.014,t=3.272,P=0.002,P=0.012,P=0.023,P=0.010). Compared with aggravation subgroup, indicators of pulmonary function in remission sub-group after treatment were increased significantly, level of Fbg was increased significantly (t=5.404,t=3.984,t=4.026,t=4.368,P=0.000,P=0.017,P=0.013,P=0.007). After treatment, the indicators of blood gas in COPD group and ACOS group were obviously improved(P<0.01).the levels of PaCO2, PaO2, SaO2 in COPD group were significantly better than those in ACOS group (t=8.203,t=3.310,t=4.492,P=0.000,P=0.032,P=0.000). Before treatment, level of Fbg in COPD and ACOS group was negatively correlated with PEF, FEV1, FEV1/pred (COPD group:r=-0.508,r=-0.574,r=-0.601,P=0.017,P=0.012,P=0.009;ACOS group:r=-0.529,r=-0.578,r=-0.621,P=0.019,P=0.010,P=0.007). After treatment, level of Fbg in ACOS group was negatively correlated with PEF, FEV1, FEV1/pred (r=-0.531,r=-0.543,r=-0.564,P=0.020,P=0.021,P=0.011). Conclusion ACOS patients have a higher pre-thrombotic state, and the detection of plasma Fbg can be helpful for early diagnosis and guiding treatment of ACOS.关键词
