调强适形放疗联合替吉奥治疗食管癌术后局部复发的临床观察

Abstract
Objective To observe the clinical efficacy and safety of intensity modulated radiation therapy(IMRT) combined with S-1 for local recurrence after esophagectomy in patients with esophageal cancer.Methods A total of 46 patients with local recurrence after esophagectomy were randomly divided into concurrent chemo-radiotherapy group(CCRT) and control group(CRT),23 cases in each group.Both groups were treated with intensity modulated radiation therapy (DT:50.4～60.0 Gy,1.8～ 2.0 Gy/Fx,5 times every week);the CCRT group were given S-1 orally 40 mg/m2 twice everyday for consecutive 14 days,and one cycle lasted for 21 days.Therapeutic efficacy,adverse reactions and long-term efficacy were analyzed and compared after one month.Results The response rates of CCRT group and CRT group was 91.3％ and 65.2％,respectively,and there was statistically differences between two groups(P＜0.05).One-year survival rates of two groups was 73.9％ vs.43.5％ (P＜0.05).No adverse reaction of hematology above grade ⅣV and degree 3 to 4 gastrointestinal reaction was observed,but the incidences of hemnatology toxicity and gastrointestinal reaction were significant differences between two groups (P＜0.05).Conclusion IMRT combined with S-1 could improve the clinical efficacy,the adverse reactions in the gastrointestinal treatment group was increasd but which still could be tolerated.关键词
