某院2015年新生儿万古霉素血药浓度的监测及应用分析
Monitoring of Plasma Vancomycin Concentration in Neonates in a Hospital in 2015 and Its Application Analysis

Abstract
Objective To analyze the plasma concentration monitoring and clinical application of vancomycin in a hospital,and provide the basis for clinical rational administration.Methods The clinical data of neonatal patients who recieved vancomycin and plasma concentration monitoring during 2015 were extracted from neonatal department in our hospital by clinical pharmacy workstation.The general information,etiological examination,results of plasma concentration monitoring,drug combination,hepatic and renal function were retrospectively summarized and analyzed.Results Among the 56 neonates,25 cases (44.64％) were firstly diagnosed as neonatal pneumonia.The ratio of samples that underwent etiological examination was 96.43％ and the postive rate was 71.43％,56.25％ neonates had a trough concentrations range of vancomycin between 5 and 10 μg · mL-1.The mean duration of medication was (12±3) d,the total effective rate was 75％.There was no obvious adverse reactions and hepatic,renal function lesion during the treatment period.Conclusion Due to the obvious individual differences of plasma vancomycin concentration,personalized medicineshould be performed and the drug dose should be adjusted according to the blood concentration monitoring and hepatic,renal function.关键词
