Abstract
Objective To study the effect of continuous care model on elderly patients with chronic diseases.Methods Admitted to our hospital from 2016.2-2017.2 months in 116 cases of elderly patients with chronic diseases,according to the order of admission,the patients were divided into normal group of 58 cases and 58 cases in the intervention group and normal group received clinical nursing process in general hospital,patients in the intervention group received continuous nursing clinical services,summarize the two groups of patients with health knowledge the evaluation scores,the scores on self-care ability of patients.Results The scores of health knowledge index in the general group were lower than those in the intervention group,P<0.05.After nursing intervention,the intervention group was higher than the normal group,P<0.05.Conclusion For the elderly patients with chronic diseases in the hospital care provided on the basis of continuous nursing mode,the higher the health knowledge score after nursing intervention in most patients,and who follow up after treatment in patients with good compliance,good self-care ability.关键词
