心脏术后患者输注回收式自体血的临床观察
The application of salvaged autologous blood transfusion after cardiac surgery

Abstract
Objective To investigate blood gas changes and clinical effect of intra-operative autologous blood salvage and transfusion after cardiac surgery.Methods Salvaged transfusion blood of 60 patients undergoing cardiac surgery was stored at room temperature.Then blood samples were taken at 1 h,3 h,5 h,7 h,9 h and 12 h to do the blood gas and the parameters were collected,such as heart rate (HR),mean artery pressure (MAP),hemoglobin (Hb),red blood cell specific volume (Hct),activated cloting time (ACT) and prothrombin time (PT) before and after autologous blood transfusion respectively.The hemoglobin in patients urine samples were monitored within 10 hours after the autologous blood transfused.Results There was no significant changes of Hb,Na+,Ca2+,CL-in 12 h.With the extension of storage time,hydrogen ion concentration (PH) value gradually decreased,K+ and Lac concentration gardually increased (P<0.05).After transfusion,Hb and Hct were greatly improved (P<0.05),while the MAP,ACT and PT had no statistical significant change (P>0.05).Only 1 patient's result of uHB testing showed weakly positive and the hemoglobinuria ratio was 1.66%.Conclusion The salvaged blood transfusion would change significantly at room temperature with time.In clinical practice,salvaged blood transfusion should be controlled in 5 hours.Intra-operative autologous blood salvage and transfusion is very useful on blood conservation,which can salvage the blood effectively and be feasible after cardiac surgery.关键词
