Abstract
Objective To investigate the diagnostic value of transvaginal three-dimensional ultrasound C planar imaging in the diagnosis of cornual pregnancy. Methods From February 2014 to July 2016, 103 cases suspected of cornual pregnancy received transvaginal two-dimensional and three-dimensional ultrasound C plane examination in our hospital. The uterine horn on the C plane, endometrium on the left and right sides of gestational sac, and lateral muscle layer surrounding gestational sac were mainly observed. In all cases, the results of preoperative ultrasonic diagnosis were compared with the results after hysteroscopy, laparoscopy and laparotomy. Results Through operation, 94 cases were diagnosed with cornual pregnancy, 5 cases with intrauterine pregnancy, 3 cases with interstitial tubal pregnancy and 1 case was diagnosed with isthmus pregnancy. The accuracy rate of three-dimensional combined with two-dimensional ultrasonography in diagnosis of uterine horn pregnancy was 94.6% (89/94). Misdiagnosis of interstitial pregnancy occurred in 5 cases. The diagnoses of partial angular intrauterine pregnancy in 5 cases and isthmus pregnancy in 1 case were consistent with the results of surgical diagnosis. The accurate rate of two-dimensional ultrasonography for diagnosis of cornual pregnancy was about 78.7% (74/94), 26 cases were misdiagnosed as interstitial pregnancy. The two methods had significant difference in the accuracy of diagnosis of cornual pregnancy (X2=13.1,p <0.001). Conclusions The accuracy of transvaginal three-dimensional ultrasound C planar imaging combined with two-dimensional ultrasonography in the diagnosis of uterine horn pregnancy is significantly better than that of transvaginal two-dimensional ultrasonography alone. Compared with hysteroscopy, laparoscopy and open surgery, it is a non-invasive, time-saving and economic examination method, and has important guiding significance for the choice of clinical treatment of uterine horn pregnancy.关键词
