降落伞式、单荷包缝合及双荷包缝合在重度混合痔PPH中的应用研究

Abstract
Objective To compare the effect of parachute type,single and double purse string suture in procedure for prolapse and hemorrhoids (PPH)in severe mixed hemorrhoids.Methods 135 patients with severe mixed hemorrhoids were randomly assigned to A,B,C groups,with 45 cases in each group.All groups were treated by PPH.Group A received parachute type suture,group B single purse string suture,and group C double purse string suture.The width,integrity and uniformity of rectal mucosal resection,incidence rates of postoperative bleeding,pain,anastomotic stenosis and anal bulge and the rate of hemorrhoids prolapse again during 6 months of follow-up were compared among the three groups.Results The width,integrity and uniformity of rectal mucosal resection in group A [(3.65±0.72)cm,97.78％ and 93.33％,respectively] were higher than those in group B and group C (P ＜ 0.05).The incidence rates of postoperative pain,bleeding and anastomotic stenosis were similar in the three groups,while the incidence rate of anal bulge in group A was lower than that in group B (P ＜ 0.05).During the follow-up,there was 1 case (2.22％)with recurrent hemorrhoids prolapse in group A,which was lower than that in group B (P ＜ 0.05).Conclusion The parachute type string suture for PPH can increase the width of resection,with better integrity and uniformity in the treatment of severe mixed hemorrhoids.It can reduce the recurrent hemorrhoids prolapse.It is a preferred suture method for PPH.关键词
