腹腔镜在全结肠系膜切除术治疗右半结肠癌中的应用体会

Abstract
Objective To study the value of laparoscopic total mesorectal excision in the treatment of the right-side colon cancer.Methods 80 patients with colon cancer were randomly assigned into two groups,with 40 cases in each group.The treatment group underwent laparoscopic CME while the control group were treated with open CME.The general situation of surgery,therapeutic effect and postoperative complications of the two groups were compared.Results The intraoperative blood loss,postoperative 24 h drainage and total complication rate of the treatment group were significantly lower than those in the control group (P ＜ 0.05).There was no significant difference between the two groups of lymph nodes and the operation time (P ＞ 0.05).The rating of excellent and good quality of life in the treatment group was 92.5％,which was higher than that in the control group (75.00％,P ＜ 0.05).The total complication rate was 10.00％ in the treatment group and 50.00％ in the control group (P ＜0.05).The 3-year overall survival rate and the disease-free survival rate were 70.00％ and 47.50％,respectively,in the treatment group and 62.50％ and 37.50％,respectively,in the control group.There was no significant difference between the two groups (P ＞ 0.05).Conclusion The laparoscope CME has equal curative effect compared with open CME in the treatment of right-side colon cancer.It can achieve better anus preservation rate,fast recovery and has easy access to patient.It can also improve quality of life.关键词
