中西医辨证治疗乙型肝炎病毒慢加急性肝衰竭
Traditional Chinese medicine and western medicine in the treatment of chronic hepatitis B patients with acute liver failure

Abstract
Objective To investigate the clinical effect of Chinese and western medicine on chronic hepatitis B patients with acute liver failure (ACLF-HBV) and its effect on liver,kidney and blood coagulation.Methods 86 ACLF-HBV patients were randomly divided into two groups.The control group (n=43) received entecavir,the treatment group (n=43) rcceived Liangxue Jiedu decoction at the same time,both were treated for eight weeks.Liver function,blood coagulation,renal function,serum virology indexes and AFP were detected and compared before and after treatment.Results After treatment,the GTP,ALT,AST,TBIL of two groups were lower (P＜0.01),and the ALT,TBIL levels in treatment group were lower than those in control group (P＜0.05).After treatment,the PTA increased,INR decreased in two groups;BUN,Cr in treatment group had no significant change,and which were lower than before treatment in control group;After treatment,the coagulation and renal function indexes in two groups were significantly different (P＜0.05 or P＜0.01).After treatment,HBeAg,HBV DNA and AFP levels in two groups were lower than before treatment (P＜0.01),HBeAg,AFP levels in treatment group were significantly lower than in control group (P＜0.01),there was no significant difference between two groups in HBV DNA (P ＞ 0.05).Conclusion The TCM syndrome differentiation combined with western medicine treatment of ACLF-HBV has better improve on liver and coagulation function than simple western medicine comprehensive treatment,this method will not aggravate the kidney load.关键词
