超声引导法与定位法在重症病人颈内静脉置管术中应用效果比较
A COMPARISON OF ULTRASOUND-GUIDED AND ULTRASOUND-LOCATED METHODS IN INTERNAL JUGULAR VEIN CATHETERIZATION FOR CRITICALLY ILL PATIENTS

Abstract
Objective To compare the application of ultrasound-guided and ultrasound-located in internal jugular vein catheterization (IJVC),in order to provide reference for clinical selection of appropriate technique for this procedure.Methods Using ultrasound-guided and ultrasound-located technique,98 critically ill patients were selected for IJVC,respectively.Number of puncture,operation time,time of indwelling catheter and complicatios were recorded and analyzed statistically.Results The difference in average operating time between ultrasound-guided group and ultrasound located group was statistically significant (t =-2.228,P ＜0.05),and the differences in number of puncture and time of catheter indwelling between the two groups were not significant (P＞0.05).No complications were noted in the ultrasound-guided group,and in the ultrasound-located group,the internal carotid artery was punctured by mistake in two cases,but the difference in complications between the two groups was not statistically significant (P＞0.05) Conclusion Applying ultrasound-guided for internal jugular vein catheterization,the puncture tip path can be observed in real time,this procedure is suitable for patients with abnormal anatomy in internal jugular vein;and applying the ultrasound-located method,it has the advantages of shorter operation time and simple,which is suitable for patients requiring rapid catheterization.As for the operators,a reasonable selection of ultrasound assisted method is needed according to the surgeon's familiarity with the technique.关键词
