Abstract
Objective To study the effects of damage control in the treatment of multiple traumas of bone and joint.Methods A total of 172 patients with multiple traumas of bone and joint were selected and divided by early treatment plan into a damage control group(n=77) and an early operation group(n=95).Bleeding control,temporary fixation,simple surgery to solve dangerous damage and other plans were conducted on the damage control group to control the damage;internal fixation was carried out when the vital signs got stable;definitive internal fixation was conducted on the early operation group immediately after diagnosis,with complications treated synchronously.The effects of operation,outcome and complication treatment between the two groups were compared.Results (1)The mean operation time in the damage control group (66.9±12.5)min was shorter than that in the early operation group (172.5±43.2)min,and the intra-operative blood transfusion (1372.5±452.5)ml was less than that in the early operation group (1953.5±377.5)ml;the success rate of operation was 97.3％(218/224),higher than that in the early operation group 83.9％ (261/311),showing statistically significant difference (P ＜ 0.05).(2)There was one pre-operative death and two post-operative deaths in the damage control group,showing a death rate of 3.9％;there were 13 innra-operative and post-operative deaths in the early operation group,showing a death rate of 13.7％.There was significant difference in the death rate between the two groups(P ＜ 0.05).(3)Damage complications could be effectively controlled during the operation in the two groups,and the complications in the damage control group were fewer than those in the early operation group (P ＜ 0.05).Conclusion For the patients with multiple traumas of bone and joint,early control of various complications and implementation of definitive surgical treatment when the patients' vital signs get stable can effectively reduce the impact of damage complications on the operation,improve intra-operative and prognostic outcomes,lower post-operative complication rate and death rate,and is worthy of preferred application.关键词
