经多裂肌入路与经正中后入路治疗胸腰椎骨折的临床效果对比

Abstract
Objective: To explore the application value of different approaches in the treatment of thoracolumbar fractures.Methods: 78 cases with thoracolumbar fractures in our hospital from May 2013 to September 2015 were included and divided into Group A and B with 39 cases in each group by random number table.All cases underwent pedicle screw fixation for the treatment, Group A used multifidus muscle approach while Group B used posterior median approach.The features of operation, improvement in pain and injured vertebra recovery were observed.Methods: The operation time of Group A was significantly shorter than of Group B, the intraoperative hemorrhage of Group A was significantly less than of Group B, the difference is statistically significant (P<0.05).The preoperative pain in 2 groups showed no significant difference (P>0.05).VAS score in Group A at 3rd day after operation was lower than in Group B, the difference is statistically significant (P<0.05).The preoperative Cobb angle and anterior vertebral height in 2 groups showed no significant difference (P>0.05), these indexes were significantly improved 6 months after operation, but differences between groups were not statistical (P>0.05).Conclusion: The multifidus muscle approach and posterior midline approach in the treatment of thoracolumbar fractures can both achieve satisfactory results, but the trauma is less in former, and operation is more simple, postoperative pain recovery time is shorter, it is worth promoting.关键词
