妊娠第7周超声检查指标和血清孕酮水平对自发性流产的预测价值研究
Value of Ultrasonic Testing Indicators and Serum Progesterone Level at the 7th Week of Pregnancy in Predicting Spontaneous Abortion
摘要
Abstract
Objective To explore the value of ultrasonic testing indicators and serum progesterone level at the 7th week of pregnancy in predicting spontaneous abortion.Methods A total of 220 pregnant women building files in Lishui Maternal and Child Care Service Center from January 2013 to December 2015 were selected,all had a gestational age of 7 weeks.Transvaginal ultrasound was conducted to collected fetal heart rate(FHR),mean gestational sac diameter(MGSD),and crown-rump length(CRL) at the 7th week,and to calculate the ratio of MGSD/CRL.Meanwhile serum progesterone levels were detected.According to the later pregnancy outcome,pregnant women were divided into spontaneous abortion group(n=34) and non-spontaneous abortion group(n=186).The ultrasonic testing indicators and serum progesterone level were compared between two groups,and the values in predicating spontaneous abortion were analyzed.Results There was no significant difference in age,gestational age,CRL and serum progesterone levels between the two groups(P>0.05).The FHR,MGSD and MGSD/CRL in spontaneous abortion group were significantly lower than those in non-spontaneous abortion group(P<0.05).Multivariate Logistic regression analysis showed that FHR,MGSD and MGSD/CRL were the influencing factors of spontaneous abortion in pregnant women(P<0.05).The results of the receiver operating characteristic(ROC) curve showed that the critical point of FHR,MGSD and MGSD/CRL in predicating spontaneous abortion was 111.15 times/min,16.00 mm,11.27 respectively,their sensitivity was 98.4%,78.0%,93.0% respectively,specificity was 88.2%,100.0%,91.2% respectively and ROC area under the curve(AUC) was 0.863,0.745,0.848 respectively.The sensitivity,specificity and AUC of combined application of these three in predicating spontaneous abortion were 89.2%,91.2% and 0.948 respectively.Conclusion At the 7th week of pregnancy,FHR,MGSD,MGSD/CRL and the combination of these three have relatively good value in predicting spontaneous abortion.关键词
