自发性细菌性腹膜炎患者可溶性髓样细胞触发受体1表达研究
Expression of Soluble Triggering Receptor Expressed on Myeloid Cell-1 in Spontaneous Bacterial Peritonitis

Abstract
Objective To investigate the expression of soluble triggering receptor expressed on myeloid cell-1 (sTREM-1) in spontaneous bacterial peritonitis(SBP).Methods The participants enrolled in this study were 23 cirrhotic patients with ascites and SBP(SBP group),and 66 cirrhotic patients with ascites but without SBP(non-SBP group),admitted in Department of Gastroenterology,Shaoxing People′s Hospital from June 2013 to December 2014.Based on the severity of SBP,patients in the SBP group were divided into mild SBP subgroup(14 cases) and severe SBP subgroup(9 cases).White blood cells(WBC) count,C-reactive protein (CRP),procalcitonin(PCT) and sTREM-1 in serum and exfoliated cells,polymorphonuclear leukocytes(PMN) count,lactate dehydrogenase(LDH) and sTREM-1 in ascitic fluid were measured in all the patients.Results SBP group had higher WBC,CRP,PCT and sTREM-1 in serum and exfoliated cells,PMN count,LDH and sTREM-1 levels in ascitic fluid than non-SBP group did(P<0.05).Ascitic fluid bacterial culture results were negative in all the patients in non-SBP group,but positive in 56.5%(13/23) of the patients in SBP group.WBC,CRP,PCT and sTREM-1 in serum and exfoliated cells,PMN count,LDH and sTREM-1 levels in ascitic fluid did not differ significantly between mild SBP subgroup and severe SBP subgroup(P>0.05).Pearson rank correlation coefficient analysis found that,serum sTREM-1 level was positively correlated with ascitic fluid sTREM-1 level in SBP group(r=0.737,P<0.001) and non-SBP group(r=0.275,P=0.025).Conclusion The sTREM-1 levels elevated in both the serum and ascites in cirrhotic patients with ascites and SBP,and they did not vary with the severity of SBP.Ascitic and serum sTREM-1 can be a reliable predictor for SBP.关键词
