术前减黄对肝门部胆管癌手术效果的影响
Effect of preoperative biliary drainage on the surgical outcome of hilar cholangiocarcinoma

Abstract
Objective To investigate the effect of percutaneous transheaptic cholangid drainge (PTCD) on the surgical outcome of hilar cholangiocarcinoma (HC). Methods From Jun. 2013 to Jun. 2016, 33 cases of con-secutive patients with HC who underwent hemihepatectomy in Jinan Military General Hospital were prospective-ly analyzed. According to the including/excluding criteria, 33 patients were randomized divided into PBD group (n=16) or non-PBD group (n=17). The difference of surgical outcome between the two groups was compared. Results The total bilirubin level [(26.4±7.5)μmol/L vs (308.6±145.2)μmol/L] and alanine aminotransferase level [(60.1±26.4) U/Lvs (525.8±257.5)U/L] were decreased significantly after PBD (P<0.05); the operation duration [(5.6±0.8)h vs (6.5±1.2)h], intraoperative blood loss volume [(560.7±159)mL vs (685.3±188.2)mL], blood trans-fusion volume [(398.1±101.5)mL vs (523.5±139.3)mL] and the incidence of postoperative complications were significantly reduced (31.3% vs 100%) in PBD group (all P<0.05). Conclusion Our study suggests that PBD can improve preoperative liver function, shorten the operation time, reduce the amount of intraoperative blood loss and postoperative blood transfusion, and PBD can reduce the incidence of postoperative complications in HC pa-tient who need hemihepatectomy.关键词
