Abstract
Objective To observe the effect of ulinastatin on C-reactive protein (CRP), interleukin-6 (IL-6), oxygenation index, partial pressure of carbon dioxide (PaCO2) and alveolar-arterial oxygen pressure difference [P(A-a)DO2] in children with congenital heart disease surgery.Methods Forty cases of children with congenital heart disease visiting Jinhua People's Hospital from March 2013 to June 2015 were randomly selected and divided into control group and experimental group according to treatment method with 20 cases in each group.Based on conventional surgical repair of ventricular septal defect, the experimental group was given ulinastatin and the control group was given same amount of saline.Inflammatory indexes and blood gas analysis were observed and analyzed before operation (T0), after cardiopulmonary bypass (T1), at 2h after operation (T2) and 1d after operation (T3).Results Differences in CRP level between two groups at T0 and T1 were not statistically significant (both P>0.05), and CRP level increased significantly at T2 and T3 in both group, which had significant difference compared to that at T0 (in experimental group, t value was 8.702 and 10.380, respectively, both P<0.05;in control group, t value was 16.363 and 13.337, respectively, both P<0.05).CRP level in the experimental group was significantly lower than that in the control group at T2 and T3 (t value was 3.815 and 2.684, respectively, both P<0.05).IL-6 level at T1 and T2 in both groups were significantly higher than that at T0 (in experimental group t value was 2.390 and 11.695, respectively, both P<0.05;in control group t value was 6.488 and 19.574, respectively, both P<0.05), and that at T3 decreased significantly in both groups.But IL-6 level in the experimental group was significantly lower than that in the control group at T1, T2 and T3 (t value was 3.329, 7.400 and 7.634, respectively, all P<0.05).Oxygenation index in two groups was significantly decreased at T0-T2, but that in the experimental group was significantly higher than in the control group at T1-T3 (t value was 9.063, 8.405 and 3.830, respectively, all P<0.05).P(A-a)DO2 in two groups was significantly increased at T0-T2, but that in the experimental group was significantly lower than that in the control group at T1-T3 (t value was 4.918, 5.011 and 6.306, respectively, all P<0.05).PaCO2 in two groups had no obvious change and difference was not statistically significant (all P>0.05).There was no significant difference in incidence of adverse reactions between two groups (χ2=1.441, P=0.230).Conclusion Ulinastatin can effectively inhibit inflammatory response and improve pulmonary oxygenation function significantly in children with congenital heart disease surgery.关键词
