3种内膜准备方案对多次胚胎移植失败患者冻融胚胎移植结局的影响
Effects of three kinds of endometrial preparation on outcomes of frozen thawed embryo transfer in patients with multiple embryo transfer failure

Abstract
Objective To explore the effects of natural cycle, hormone replacement cycle and down regulated hormone replacement cycle on outcome of frozen thawed embryo transfer in patients with multiple embryo transfer failure.Methods Altogether 130 patients with multiple embryo transfer failure meeting inclusion criteria and hospitalized in reproduction medicine center in Hainan Maternal and Child Health Care Hospital and department of reproduction and genetics in Suzhou Hospital Affiliated to Nanjing Medical University from December 2015 to December 2016 were selected and divided into natural group (n=51), replacement group (n=32) and down regulation group (n=47) according to different endometrial preparation program.Retrospective analysis was made on samples and general clinical data of patients such as age, number of thawed embryos, infertility duration, number of embryos transferred, endometrial thickness on day of conversion and level of estradiol were analyzed and compared.In natural cycle and during frozen thawed embryo transfer, endometrial morphology on day of conversion was compared among three groups and pregnancy outcomes such as embryo implantation and early abortion of patients in three groups were recorded.Results There was no significant difference in age, number of thawed embryos, infertility duration, number of embryos transferred, endometrial thickness on day of conversion and estradiol level of patients in three groups (all P>0.05).Endometrial morphology in natural group in natural cycle and during frozen thawed embryo transplantation was better than that in replacement group and down regulation group (χ2 value was 11.99 and 14.88, respectively, both P<0.05).Pregnancy rate and implantation rate in down regulation group were 53.19% (25/47) and 61.70% (29/47), respectively, which were significantly higher than those in replacement group and natural group (χ2 value was 9.80 and 11.80, respectively, both P<0.05), but early abortion rate in three groups had no significant difference (χ2=3.51, P>0.05).Conclusion Pregnancy rate and embryo implantation rate of patients with multiple embryo transfer failure could be effectively improved by hormone replacement cycle after down regulation.For patients with endometrial defects or poor status, hormone replacement cycle after down regulation is preferred.关键词
