胆汁回输对胰十二指肠切除患者术后并发症及营养状态的影响
The influence of bile reinfusion on postoperative complications and nutritional status in patients after pancreaticoduodenectomy

Abstract
Objective:To explore the influence of bile reinfusion on postoperative complications and nutritional status in patients after pancreaticoduodenectomy (PD).Methods:62 patients who received PD in our Hospital from January 2014 to January 2016 were selected and divided into a research group (bile reinfusion combined with enteral and parenteral nutrition,n =30) and a control group (only enteral and parenteral nutrition,n =32).The postoperative complications incidence and nutritional status between the two groups were compared.Results:The incidence of diarrhea and delayed gastric emptying,the hospital days and central venous extubation time were lower in the research group than those in the control group (P ＜ 0.05).On day 5 after PD,the albumin(ALB) level was similar to that at pre-operation (P ＜0.05),while the prealbumin (PALB),retinol-binding protein (RBP) and transferrin (TRF)levels were markedly decreasedin both groups (P ＜0.05).On day 10 after PD,the body mass index (Ibm),triceps skin fold thickness (SFT),midarm circumference (MAC) and corrected arm muscle area (CAMA) were significantlyreducedin both groups (P ＜0.05).The Ibm,SFT,MAC and the levels of PALB,RBP and TRF in the research group were significantly higher than those in the control group (P ＜0.05).On day 30 after PD,the Ibm and MAC in the research group were also higher than those in the control group(P ＜0.05).Conclusion:Bile reinfusion could reduce delayed gastric emptying,reduce diarrhea incidence,shorten hospital days and central venous extubation time,and improve postoperative nutritional statusin PD patients.关键词
