超声内镜下黏膜切除术在上消化道病变治疗中的应用观察

Abstract
Objective To observe effect of endoscopic mucosal resection (EMR) by guided endoscopic ultrasonography (EUS) in treatment of upper gastrointestinal lesion.Methods A total of 108 patients with gastrointestinal protrusion lesions during January 2013 and December 2015 underwent EMR by EUS or endoscopic mucosal resection (EPMR), and therapeutic effects were observed and analyzed.Results EMR was successfully performed at by one time in 103 patients, and other 5 patients with more than 2cm diameter of lesions were completely removed by EPMR.Postoperative pathological results showed that 2 lesions of metaplastic esophageal polyps, 10 lesions of esophageal leiomyoma, 16 lesions of esophageal papilloma, 6 lesions of esophageal stromal tumor and 2 lesions of esophageal intra-mucosa cancer;2 lesions were cardial adenoma, 8 lesions of cardial hyperplastic polyps, 30 lesions of gastric adenocarcinoma, 2 lesions of gastric eosinophilic granuloma, 8 lesions of gastric hyperplastic polyps, 8 lesions of gastric stromal tumor, 5 lesions of gastric ectopic pancreas and 3 lesions of gastric lipoma;4 lesions were duodenal gland tubular adenoma combined with mild-moderate atypical hyperplasia, and 2 lesions were descendent duodenum Brunner's gland adenoma.Among the 108 patients, 6 patients had intraoperative a little bleeding, and were controlled successfully by corresponding treatment.The lesions were healed without residual or recurrent lesions by postoperative 6 months recheck for the 108 patients.Conclusion EMR by EUS guide has good effect and few complications in treatment of upper gastrointestinal lesion.关键词
