Abstract
Objective To compare the efficacy and safety of percutaneous coronary interventional (PCI) therapy and coronary artery bypass grafting (CABG) in octogenarians with unprotected left main coronary disease.Methods From January 2012 to September 2015,according to propensity score matching (PSM) method,54 octogenarians with unprotected left main coronary artery disease were enrolled in this study.Twenty-eight patients underwent PCI therapy and 26 patients underwent CABG.Major clinical adverse events (death,recurrent angina,and revascularization due to ischemia) were recorded in detail,including clinical features during one-year follow-up period.Results There were no significant differences in age,sex,smoking,hypertension,diabetes,hyperlipidemia,stroke,old myocardial infarction and previous PCI between PCI and CABG groups.When CABG group were compared with PCI group,no significant differences were found in cardiac function (left ventricular ejection fraction) (59.96%±10.75% vs 62.57%±8.20%,P=0.318),SYNTAX score(30.31±4.35 vs 29.93±4.84,P=0.764) and EuroScore Ⅱ score (3.06±0.78 vs 2.97±0.67,P=0.657).PCI group compared with CABG group,there were some differences in mortality (7.69% vs 0%,P=0.439),recurrence of angina pectoris (3.85% vs 7.14%,P=1.000),target vessel revascularization (3.85% vs 7.14%,P=1.000) and incidence of major adverse cardiac events (11.5% vs7.14%,P=0.410) but not statistically significant.Conclusions PCI is safe and effective for unprotected left main coronaryartery disease in octogenarians and can be used as an alternative to CABG.关键词
