Abstract

Objective To evaluate the efficacy of intravascular ultrasound (IVUS)-guided intervention therapy for unprotected left main coronary artery disease.Methods Three hundred and sixty eight patients with unprotected left main coronary artery disease confirmed by coronary angiography from January 2012 to December 2015 were enrolled.By propensity score matching (PSM) method and with the tendency of 1:2,36 patients were selected for the IVUS-guided IVUS group and 72 patients for the conventional intervention therapy group (CAG group).The clinical characteristics and major adverse cardiac events (MACE) (including death,recurrent angina and re-vascularization of target vessel) during hospitalization and one year follow-up were compared.Results There were no significant differences between the two groups in age,gender,smoking,hypertension,diabetes,hyperlipidemia,stroke,old myocardial infarction,history of percutaneous coronary intervention (PCI) therapy and history of peripheral vascular disease(all P>0.05).The minimal lumen diameter [(1.06±0.42) mm vs (1.01±0.39) mm,P=0.697],stenosis of lumen diameter (83.9%±10.4% vs 87.6%±9%,P=0.064),mortality (0% vs 2.8%,P=0.551),recurrence rate of angina (8.3% vs 25%,P=0.071),target vessel re-vascularization rate (5.6% vs 13.9%,P=0.330) also showed no significant differences.However,compared with the CAG group,the reference vessel diameter [(3.53±0.52) mm vs (3.29±0.47) mm,P=0.018],stent diameter [(3.72±0.34) mm vs (3.44±0.41) mm,P=0.001],maximum non-compliance balloon diameter [(3.99±0.43) mm vs (3.54±0.47) mm.P=0.000)] and minimal lumen diameter after PCI [(3.88±0.40) mm vs (3.49±0.42) mm,P=0.000) in IVUS group were significantly larger,the incidence of MACE (8.3% vs 27.8%,P=0.038) was significantly lower.Conclusions IVUS-guided intervention therapy for unprotected left main coronary artery disease is safe and effective,and can improve the prognosis of these patients.
