Abstract
Objective To analyze the risk factors of anastomotic leakage after D2 lymphadenectomy for the patients with gastric cancer. Methods A total of 237 consecutive patients with gastric cancer who underwent gastrectomy with D2 lymphadenectomy between September 2008 and August 2009 were studied respectively. Univariate analysis was done with the clinical data including age, sex, operation time, gastrectomy type, anastomosis type, diabetes, preoperative and first postoperative day haemoglobin, preoperative and first postoperative day serum albumin and tumor stage. Multivariate Lo-gistic regression analysis was performed furthermore. Results There were leakage group of 13 cases and without leakage group of 224 cases for the univariate analysis. It was shown by univariate analysis that there was no difference between two groups in age, sex, operation time, gastrectomy type, diabetes, preoperative haemoglobin, preoperative and first postopera-tive day serum albumin, T-stage, N-stage and TNM stage of gastric cancer. However, both anastomosis type (P=0.013) and first postoperative day haemoglobin (P=0.002) were significantly different between two groups. Using Logistic regression analysis we found that first postoperative day haemoglobin (P=0.038) was the only independent risk factor to affect anasto-motic leakage after gastrectomy with D2 lymphadenectomy. Conclusions Postoperative anemia would be important to affect the anastomotic leakage after gastrectomy with D2 lymphadenectomy.关键词
