大肠侧向发育型肿瘤的结肠镜下诊治研究
Diagnosis and Treatment guided by Endoscopy for laterally spreading tumor of the Large Intestine

Abstract
Objective To investigate the value of endoscopy in guiding diagnosis and treatment for laterally spreading tumor (LST) of the large intestine.Methods 42 cases with LST of the large intestine diagnosed by the electronic enteroscope in our hospital from March 2015 to December 2016 were selected.The general morphology and pit patterns were observed through the submucosal staining and magnifying colonoscopy and compared with the pathological results.The infiltrating level were estimated by endoscopic ultrasonography,patients were treated with endoscopic mucosal resection or endoscopic submucosal dissection and the curative effects were recorded.Results Under the endoscope,LST lesions were generally divided into granular type (homogeneous type,nodular-mixed type) and non-granular type (fiat-elevated type,pseudo-depressed type).Nodular-mixed type was the most common.Through magnifying colonoscopy,lesions were mainly occupied by type Ⅲ and type Ⅳ.Through endoscopic therapy,lesions were removed clearly with little bleeding and without postoperative perforation,with favorable wound healing.No recurrence was observed during the postoperative follow-up.Conclusion Electronic endoscope provides valuable information for diagnosis of LST of the larger intestine.Endoscopic mucosal resection or endoscopic submucosal dissection can effectively manage the disease with efficacy and safety.关键词
