双撕囊联合前部玻璃体切除术在婴幼儿先天性白内障手术中的应用
Application of double capsulorrhexis combined with anterior vitrectomy in congenital cataract surgery of infants and young children

Abstract
Objective To analyze the clinical effect of double capsulorrhexis combined with anterior vitrectomy in the treatment of congenital cataracts. Methods A total of 118 eyes of 72 patients with congenital cataracts aged from 15 days to 4 years were enrolled in the research. Patients old than 2 years had posterior chamber intraocular lens implanta-tion. In Group Ⅰ, phacoemulsification was performed in 46 eyes of 28 patients. In Group Ⅱ, phacoemulsification com-bined with anterior and posterior capsulorrhexis and anterior vitrectomy was performed in 72 eyes of 44 patients. The visu-al axis opacification and various complications were followed up for 4 months to 66 months. Results In total, 103 eyes were followed up. Postoperatively, the best-corrected visual acuity was0.05to0.1in3(6.52%)and 0 eyes in Groups Ⅰ and Ⅱ, respectively; 0.1 to 0.2 in 10 (21.74%)and 1 (1.75%)eyes in Groups Ⅰ and Ⅱ, respectively;0.3 to 0.4 in 16 (34.78%)and 20(35.09%)eyes in Groups Ⅰ and Ⅱ, respectively; and >0.5 in 17 (36.96%) and 36(63.16%)eyes in Groups Ⅰ and Ⅱ, respectively. These differences were statistically significant (x2 = 4.18, <0.05). Grade >3 posterior capsular opacification was found in 11 eyes (23.92%)in Group Ⅰ and 4 eyes (5.56%)in Group Ⅱ. This difference was also statistically significant (x2 = 5.36, < 0.05). Conclusion Double capsulorrhexis combined with anterior vitrectomy in congenital cataract surgery of infants and young children can effectively prevent posterior capsule opacification and im-prove visual function.关键词
