意识模糊评估方法在痴呆患者中应用的信效度
Reliability and validity of Confusion Assessment Method Chinese Revision applied in dementia patients

Abstract
Objective To evaluate the applicability of Confusion Assessment Method Chinese Revision (CAM-CR) in hospitalized dementia patients.Methods The delirum status of hospitalized dementia patients was assessed with 11 item CAM-CR.The structural validity of the scale was evaluated by using the factor analysis;the diagnostic validity of the scale was evaluated using the DSM-Ⅳ diagnostic criterion for delirium as the gold standard.The reliability of the scale was evaluated by the internal consistency test and consistency test of evaluators.Results The factor analysis eliminated two items,then two factors model CAM scale was formed.With 24 points as the cut-off value for diagnosis of delirium,the sensitivity and specificity were 0.97 and 0.89,respectively;the coefficient Kappa value of clinical diagnosis was 0.74.The consistency ICC for the entries and the total score of the CAM-CR was between 0.80～0.91;the Cronbach-α coefficient of the total CAM was 0.71,the Cronbach-α coefficient of the two factors was 0.75 and 0.69.Conclusion The revised CAM has good reliability and validity,which is suitable for the screening delirium in dementia patients.关键词
