血糖升高对儿童手足口病重症及危重症严重度的评价意义

Abstract
Objective The aim of this study was to evaluate the clinical significance of hyperglycemia in children with hand foot and mouth disease (HFMD).Methods 336 children with HFMD were retrospectively included in this study from Apr 2014 to Sep 2014.The blood glucose,HFMD severity and clinical symptom were analyzed.And the symptoms in different stages of HFMD were compared.Results 192 cases were male (57.14%) and 144 cases were female (42.86%).The average monthly age was 30.16+20.76 months.The mean course of fever before admission was 2.98±1.67days.313 cases(93.15%),10 cases (2.98%) and 4 cases(3.87%)were in the stage of 2,3 and 4,respectively.The fever occurred in 336 cases (100%),limb shaking in 336 cases (100%),lethargy in 95 cases (28.27%),vomiting in 48 cases (14.28%),headache and delirium in 10 cases (2.98%),disturbance of consciousness in 42 cases (12.50%) and urinary retention in 30 cases (8.93%).The mean blood glucose of patients in the stage 2,3 and 4 were 6.34±1.91mmol/L,8.28±3.58mmol/L and 9.51±6.76mmol/L,respectively.The blood glucose of patients in the stage of 4 was significantly higher than that in the stage of 2 and 3 (P<0.05).The sensitivity and specificity in the stage of 2 and 3 were 60% and 74.12% with glucose as the reference index.The area under the AUC curve was 0.69.The sensitivity and specificity in the stage of 2 and 4 were 84.62% and 53.04%.The area under the AUC curve was 0.72.The sensitivity and specificity in the stage of 3 and 4 were 53.85% and 60%.The area under the AUC curve was 0.55.Conclusion It has a correlation between high blood glucose level and severity of the disease for most severe HFMD children.The blood glucose as a reference index can be used as a serological marker to evaluate the severity of HFMD,but its sensitivity and specificity is not high.关键词
