摘要

Abstract

Objective To explore the value of fluorescence real-time quantitative PCR(FQ-PCR) for detecting the bacterial pathogen of bloodstream infection(BSI)in children,and to explore the relationship between the bacteria load in primary infection site and the incidence of BSI,the positive rate of culture.Methods A total of 14 patients with Acinetobacter baumannii infection in primary infection site and suspected secondary bloodstream infections were included.The bacteria DNA was extracted and analyzed by 16S rDNA-PCR and SYBR Green FQ-PCR.The results of quantitative bacteria load and blood culture(BC) were compared between primary infection samples and blood samples.Results Among the 14 specimens (9 bronchoalveolar lavage fluid and 4 ascetic fluid),the medians (quartile)of bacteria load in primary infection sites and blood were 5.64 × 106 (3.15 × 105,4.345 × 107) CFU/ml and 1.585 × 105 (2.315 × 103,5.817 × 105) CFU/ml,respectively,and there was significant difference (Z =-2.941,P < 0.01).The medians (quartile) of bacteria load in 10 positive BC and 4 negative BC were 2.90 × 105 (1.058 × 105,1.057 × 106) CFU/ml and 1.79 × 103 (8.60 × 102,2.315 × 103) CFU/ml,respectively,and there was also significant difference (Z =-2.828,P < 0.01).Conclusion The bacteria load in primary infection site is higher than that in blood,and blood culture might be positive if the bacteria load is > 105 CFU/ml in blood.
