华法林在肾病住院患者中的用药分析
Analysis of the application of warfarin in nephropathy patients

Abstract
Objective To investigate the clinical application of warfarin in nephropathy patients of our hospital.Methods The data of inpatients using warfarin in the department of nephronia of Peking university first hospital from July 1 to December 31 in 2014 were collected,the indication,usage,target range of international normalized ratio (INR),the INR level during hospitalization,and adverse drug reactions were analyzed.Results A total of 150 inpatients who received warfarin were enrolled.The use of warfarin involoved 6 indications which was mainly used for prophylactic anticoagulation therapy of nephrotic syndrome patients whose serum albumin decreased significantly (130,86.67％).127 never used warfarin before,and their initial dose were 3 mg (65 cases),1.5 mg (52 cases),or 2.25 mg (10 cases).The maintenance dose was (2.46 ± 1.26)mg.There were some differences in the INR target range.75 patients (50.00％) with INR ＜ 1.5,103 patients (68.67％)with INR ＜ 2.0;6 bleeding events happened during therapy,the incidence was 4.00％.Conclusion The warfarin dosage and the INRs of kidney disease patients were lower.It needs further evidence and more clinical trials to decide whether pharmaceutical intervention is needed.关键词
