摘要

Abstract

Objective To investigate the application status and influencing factors of appropriate health technology in grass - roots medical workers in different parts of China. Methods A questionnaire survey was conducted in this study between July and August 2010. The procedure that we used to select the appropriate subjects is as follows:in accordance with the status of economic development,disease distribution characteristics,disease prevention and treatment needs and characteristics,we firstly extracted 4 - 5 provinces (municipalities and autonomous regions)from eastern,central and western China,then,from each of the places,we selected 1 - 2 counties in which appropriate health technology for grass - roots medical workers were extended and demonstrated. And from the medical institutions in these counties,such as county hospital,county hospital oftraditional Chinese medicine,county maternal and child health care hospital,township hospital,village clinic and so on,we selected the medical workers who were on duty on the survey day and had participated in the training of appropriate health technology for grass - roots medical workers as the subjects of this study. The questionnaire used in the survey involved in the baseline characteristics of the medical workers,training and application status of the appropriate health technology in grass - roots medical workers. A total of 833 medical workers filled in the questionnaires,but only 717 returned responsive ones,with a response rate of 86. 1% . Results The utilization rate of appropriate health technology learned from the training in 717 subjects from different parts of China,in 288 subjects from eastern China,227 subjects from central China and 202 from western China was respectively 79. 9% (573 / 717 ),83. 3% (240 / 288 ),75. 3% (171 / 227 )and 80. 2% (162 / 202 ) . Multivariate Logistic regression analysis found that,for the grass - roots medical workers from eastern China,the influencing factors for the application of appropriate health technology were properties of working unit,medical professional and technical title,medical department engaged in currently,accumulated duration of training and the level of mastering trained techniques (P < 0. 05);for those from central China,the influencing factors were gender,necessity level of extending the techniques,technique training methods,the level of mastering trained techniques (P < 0. 05);for those from the western China,the correlation factor was the level of mastering trained techniques (P < 0. 05). Multivariate linear regression analysis showed that the factors affecting the application frequency of appropriate health technology in grass - roots medical workers were level of difficulties in applying the techniques,corollary equipment that using the techniques needed,and adverse reactions of patients treated with the techniques (P < 0. 05). Conclusion Application status and influencing factors of appropriate health technology in grass - roots medical workers vary from district to district of China. Therefore,it is suggested that in order to improve the mastering level and utilization rate of the techniques,the corresponding training should be held hierarchically and targetedly based on the specialties of areas and working units that the medical workers from,and the characteristics of the medical workers.
