摘要
Abstract
OBJECTIVE:To explore the role of clinical pharmacists in pharmaceutical care for patient with coronary heart disease complicated with hypertension .METHODS: Clinical pharmacists participated into the drug therapy for one patient with coronary heart disease complicated with hypertension , developed individualized pharmaceutical care plans and carried out the whole course of pharmaceutical care .RESULTS:During the treatment of coronary heart disease and hypertension , the clinical pharmacists closely cooperated with clinicians , adjusted the treatment regimen of antihypertention drugs and antithrombotic drugs , the patients discharged after getting better . CONCLUSIONS:The clinical pharmacists carry out pharmaceutical care for patient as the center can help to improve the pharmaceutical service level and service quality , ensure the rational drug use for patients .关键词
