超声引导子宫肌瘤射频热消融术与腹腔镜下子宫肌瘤剔除术的临床效果分析

Abstract
Objective:To evaluate the clinical effect of ultrasound guided radiofrequency volumetric thermal ablation (URVTA) and laparoscopic myomectomy (LM) for uterine fibroids.Methods:Two hundred and twenty patients were randomly divided into URVTA group (n=110) and LM group (n=110).To evaluate the results of follow-up,uterine fibroids recurrence (number and size of fibroids),the overall treatment effect,the use of analgesics,surgery related complications and pregnancy outcome were recorded in postoperative 3,6 and 12 months.Results:URVTA and LM patients took analgesic postoperatively for (6.5±3.6) d and (8.7±3.8) d respectively (P=0.83),missed (12.8±6.2) and (17.2±5.9) workdays (P=0.22),and resumed normal activities after (22.5±7.9) d and (28.4±7.2) d (P=0.61) respectively.In 12 months after the operation,the average size of uterine fibroids in the two groups were reduced 87.6% (URVTA) and 90.5% (LM),and 94.2% (97/103 cases,URVTA) and 96.2% (101/105 cases,LM) fibroids disappeared.Two LM patients were pregnant with one cesarean delivery and one vaginal delivery of healthy infant.Two pregnancies in the URVTA group both ended in full-term vaginal delivery of healthy infant.Conclusions:The clinical efficacy of URVTA and LM is similar in the treatment of uterine fibroid,after 12 months,the volume of menstrual blood are both decreased in the two groups,the duration of taking analgesic and the incidence of complications are similar.关键词
