Abstract
Objective To evaluate the hemostatic effect of tranexamic acid combined with different density adrenaline on total hip arthroplasty (THA).Methods Total of 156 patients who accepted unilateral THA were randomly divided into low concentration group (1 g tranexamic acid+0.125 mg epinephrine),high concentration group (1 g tranexamic acid+0.25 mg epinephrine) and control group (1 g tranexamic acid) (n=52,respectively).The solution were infused into joint capsule which were sutured to stop bleeding treatment.The total blood loss,hidden blood loss,DVT incidence rate,postoperative 3 d Hb reduction,HCT reduction,subcutaneous ecchymosis area>1% incidence rate,postoperative drainage,blood transfusion,blood transfusion ratio and postoperative half-year Harris scores were evaluated.Results All the patients operated successfully.One patient and 3 patients complained of DVT in the low concentration group and high concentration group respectively.The incidence rate of adverse reactions in the three groups was 3.85% (n=2),7.69%(n=4) and 3.85%(n=2).There was no significant difference among three groups (χ2=1.054,P=0.590).There was significant difference in total blood loss,hidden blood loss and blood transfusion among the three groups (P<0.05).Total blood loss,hidden blood loss and blood transfusion of low,high concentration group were higher than control group (t=5.417,4.448,P<0.05),but there was no significant difference between low,high concentration groups with total blood loss,hidden blood loss and blood transfusion (t=1.096,P> 0.05).At 3 d after surgery,red blood cells,Hb and HCT between groups had no significant difference (P>0.05);there were no significant differences in terms of transfusion rate,drainage volume,Hb descent,HCT value,subcutaneous ecchymosis area>1% incidence rate among the three groups.Conclusions Tranexamic acid combined with adrenaline can reduce the volume of THA bleeding.Adrenaline concentration has no effect on hemostasis,while low concentration of adrenaline may reduce the incidence of DVT.关键词
